Duplicate documents may be issued if your NI logbook and or DP Certificate have been lost or stolen.
Lost Logbook / DP Certificate
In the case of a lost logbook or DP Certificate, you will need to provide the following:
- Affidavit (sworn statement signed by solicitor or notary public) in English
- Copy of personal details page in passport
- Copy of Basic/Induction Course Certificate
- Duplicate DP Documents order form
For Black/Grey and Burgundy logbooks only:
- Passport photo
- Copy of Certificate of Competency
Stolen Logbook / DP Certificate
If your logbook and or DP Certificate has been stolen you will need to provide the following:
- Police Incident Report
- Copy of personal details page in passport
- Copy of Basic/Induction Course Certificate
- Duplicate DP Documents order form
For Black/Grey/Burgundy Offshore and Purple DPVM logbooks only:
- Passport photo
- Copy of Certificate of Competency
